Website Access Request Form
Website access requests are now submitted using our Help Desk Management ticket system. Sign into Help Desk Management, click on create ticket, and choose "Website Access Request" for the problem type.
Software Request Form
Fill out this form to request installation of NEW SOFTWARE ONLY onto district computers. For previously installed software, please submit a Help Desk Management ticket. Software on CD or other media MUST be sent to the Computer Center prior to installation so compatibility and licensing can be verified. After software installation, media will reside at the Computer Center in DMS, it will NOT be returned to you. Please be aware that any personally owned software installed on MCCSD equipment must be donated and become property of MCCSD. Proof of Licensing must be provided for EACH computer requested for installation. This form will be sent to the Director of Technology for approval.
Please note: This form is not for Google APPS or extension. Requests for those must be made through your coordinator or director to the Director of Technology.
Download this form to dispose of broken or outdated equipment in your building. Signed forms may be sent to Lisa Shevlin, Fixed Assets and Records. Please attach a typed list of the material to be disposed. This must include a description of the item, the model, the serial number, and the district property tag number.
After you've filled out the form, please drop off the Chromebook to your building library for service. If you require a loaner Chromebook, please provide TA with your ticket number so loaner information can be added to your ticket.
