Pre-Kindergarten Registration

The Westside Appointment Link will be posted here - https://www.mccsd.net/upkappt - between February 23-27, 2026 in order to make an appointment for the in-person registration in March at the Middle Country Public Library - Centereach Branch (101 Eastwood Blvd, Centereach, NY 11720).  You will be making registration appointments for March 16th through March 19th..

If you did not receive a registration packet in the mail, please visit the Registration Related Files section, at the link above, and download and print the file named Registration Packet UPK: English or Spanish.  When you arrive for your registration appointment, you will need to have this packet completed, along with required NYS documents.  You may also come to Central Registration to pick up a pre-printed packet.

What to Bring to Central Registration

Parents must bring their completed NEW ENTRANT FORM to Central Registration.
If you did not receive the District's mailed packet, contact (631)285-8890. 

You must bring the following when you come to register:

  • Completed NEW ENTRANT FORM
  • Original Birth or Baptismal Certificate (with a raised seal)
  • Custody papers (if applicable)
  • 2 Copies of Immunization Record Signed by Physician
  • Proofs of Residency

Proofs of Residency for Owners

One (1) of the following items:
Mortgage statement, Deed, property tax bill, or title
 
Two (2) of the following current items:
Utility bill, income tax form, voter registration, insurance bill, bank statement, state or government issued identification, driver’s license, learner’s permit or non-driver identification, pay stub, telephone bill, oil bill, DSS declaration or other original documents evidencing residency.

Proofs of Residency for Renters

One (1) of the following items:
Lease, sworn landlord affidavit (notarized), landlord statement (notarization optional) or unsworn third party statement, or a sworn residency affidavit (notarized).
 
One (1) of the following current items:
Utility bill, income tax form, voter registration, insurance bill, bank statement, state or government issued identification, driver’s license, learner’s permit or non-driver identification, pay stub, telephone bill, oil bill, DSS declaration or other original documents evidencing residency.

Parents with children 0 - 4 years old

If you currently have children under four years of age, please contact Central Registration at 285-8890 with your family's information regarding infants/toddlers for future mailings of pre-kindergarten and kindergarten registration information.
