Request Teacher/Principal Final Quality Rating and Composite Effectiveness Scores

To request the APPR score for your child’s teacher and/or principal, please use the appropriate form(s) linked below and mail them to the District Office.  Telephone calls, faxes or e-mails will not be accepted due to the sensitivity and confidentiality of this information.  If you are unable to access the form(s), please mail a request to the Personnel Department to have the appropriate form(s) sent to you.

The Composite Effectiveness Score is based upon a range of 0 to 100 and is the result of combining a teacher’s scores on:  (1) a State growth or comparable measures assessment; (2) a locally selected measure of growth or achievement; and (3) other measures of teacher effectiveness aligned with New York State approved Teaching Standards.  That Composite Effectiveness Score results in a Final Quality Rating using the categories provided by New York State.  An explanation of the Final Quality Rating and Composite Effectiveness Score is attached.  Additional information which may help you to understand the scores in the context of teacher evaluation and student performance may be found on the APPR links on the District’s website at: http://www.mccsd.net and from New York State on http://www.engageny.org.  
 
Release of this information by the school district to anyone other than the parent or legal guardian of a particular student is a violation of New York State Education Law.  This information is intended solely for the use of the parent or legal guardian named above and should not be disclosed to others.  This document contains information that is confidential or legally protected.  If you have received this document and you are not the parent or legal guardian named above, please contact the District Personnel Department at 631-285-8030 and discard immediately.
