  • Dear Parents/Guardians,

    If your child is going to be absent, please call the Attendance office at (631)285-8406.

    Close daily contact with many students will inevitably bring some exposure to a variety of communicable diseases, as well as any of the virus or flu strains that are common in the winter months. Watch your student for symptoms of illness before he/she comes to school. A child who is sick will not be able to perform well in school and is likely to spread the illness to other children and staff.  We suggest making a plan for childcare ahead of time so that you will have a place for your child to stay if he/she is ill.  Please be advised that students who have a fever (100 degrees or higher) are not be permitted to attend school until they have been fever free WITHOUT medication (Tylenol) for 24 hours.  Students must remain home if they have vomited within the past 24 hours.Allow for adequate convalescence after any illness, no matter how slight the illness may be. Please do not send your student to school for diagnosis or treatment by the school nurse.

    The Health Office is here to assist students who are not feeling well and or need some special treatment as per a doctor's order. It is required that all students that ask to see the nurse must have a pass. Our Nurses are always helpful and attentive as they take temperatures, give ice packs, bandages, medicines and all other health related student needs. Nurses are not permitted to make a diagnosis. A medical diagnosis must be made by your child’s physician. 

    NEW YORK STATE MANDATES THAT:

    • All Incoming 6th Grade Students are required to have the booster immunization containing tetanus, diphtheria and acellular pertussis (Tdap) as well as 3-5 doses of polio and 2 varicella (chicken pox) vaccines.  These immunizations are required prior to admission to the 6th grade. 
    • All 7th Grade Students must submit a Physical Examination Form and must provide proof of Meningitis Vaccine on or before the start of school, September 6, 2016. STUDENTS WHO DO NOT PRESENT PROOF OF IMMUNIZATION  WILL BE EXCLUDED FROM SCHOOL.
    • All Athletes must have an up to date annual physical exam in order to participate in sports.
    • All New Entrants require a physical with in thirty days of enrollment.    
    • All Working Permits require an up to date annual physical.           
       

    We are requesting that you return the medical and dental forms to the health office in September or you may mail them to the attention of the Health Office.

    Please hand in your child’s annual emergency home contact card to the Nurse’s Office.


    Medication
    If your child requires medication given during school hours, the health office requires three necessary pieces of information:

    1. A note from the parent/guardian which give the Nurses permission to give your child medicine.
    2. A Doctor's note with instructions for giving the medicine (child’s name, name of medication, times the medication is to be given, dosage to be given, and the duration of time your child is to receive medication).
    3. The medicine should be in the original sealed prescription bottle/container and handed directly to the Nurses by the parent. This includes even over the counter medication.

    Injuries
    If your child sustains an injury in or out of school, please notify the Health Office so that we can provide any assistance that your child may need.  All medical notes need to be submitted to the Health Office.  We will then distribute them to the appropriate staff members. Your child will not be permitted to resume gym/sports until a medical clearance note is given to nurse from the originating doctor.

    Sport Physicals

    You need two forms for sports participation. 

    1. An up-to-date Doctor's Health Appraisal Form
    2. Electronic Parent Permission Form from the RankOne website
      We are no longer using paper parent permission slips for sports. Please go to the Middle Country website, click on athletics and click on the RankOne sports link to make an account and complete the permission slip. Please call 285-8650 with any questions about the process.
       

    If a student wants to play sports, a sports physical from their doctor is needed (Doctor's Health Appraisal Form). If you are unable to see your private physician, physicals are offered by the visiting school physician after school on an announced day. An appointment will be made for the student to receive a physical. Students who arrive on day of the physical will not be permitted to obtain a physical. Students must listen to announcements to hear when the doctor will be in and the day the permission slips are due at the nurse's office. The papers needed for physical are available in the Health Office, Gymnasium or see the related files below for the form(s).
