    *STUDENTS MUST HAVE A PASS TO COME TO THE NURSE'S OFFICE*

    Please report all school injuries to the nurse.  All gym excuses and hall passes will be issued by the nurse with an accompanying note from the student's physician.

    EMERGENCY CONTACT CARDS

    An updated contact card is required each year.  Please update any changes in phone numbers, guardians or employment when they occur.  Central Registration must also be contacted so that your information is updated in the school-wide computer system.

    Administration of Medications in School

    In order to administer medication to your child during school, a form must be completed and signed by your physician.  A parent must also sign the form. A self-carry form is also required for students wishing to carry their own medication within limits.  See Health Forms Tab.

    Physicals

    9th and 11th graders and ALL NEW ENTRANTS are required to have state-mandated physicals.  These physicals must be performed within 30 days of the start of the school year.  Any student who does not have the mandated physical will be excluded from school. See Health Forms Tab.  In addition, 11th graders are required to have hearing and vision screening as part of their physical.

    Sports Physicals

    Any student wishing to participate in sports must have a YEARLY sports physical and PERMISSION SLIP for each sport.  Sport permission slips and physical permission slip forms are on the Middle Country Athletic Dept. website.  Permission slips must be completed and signed by a parent or guardian.  The school physician performs sports physicals three times a year prior to each sports season.
